Press Start at the main menu, then hold every other button at the same time. A new screen will appear, where you can select the team and then the player that you wish to have his name changed.

While playing press Select + Up + any of the four action buttons (X, Circle, Triangle or Square) then release Select to change the look of the players to either Very thin, Overweight, Ape-like or Tiny. Press Select + Down to deactivate.
